Carlisle Manor is a 48 bed health care facility which has been operated since 1981. Our continuing efforts at excellence have resulted in our recent deficiency free state survey, which  ranks us as not only one of the leading facilities in Warren County, but also among the leaders throughout the State of Ohio. Admissions to Carlisle Manor are coordinated by our Director of Admissions, Administrator and Director of Nursing. Upon admission our care plan team carefully designs a rehabilitation plan to meet the individual needs of each resident.

At Carlisle Manor, our residents and their families feel secure knowing that quality medical care is provided 24 hours a day, seven days a week, by caring professionals under the direction of the medical director. Our goal is to provide a plan of individualized care for our residents that will enable them to attain their highest level of independence. We offer a broad range of programs designed specifically to meet the needs of our residents, all requiring various degrees of assistance.

Carlisle Manor relies upon the combined knowledge and skill of a team of professionals to obtain that goal. Our quality staff members are carefully selected, then well trained and managed by experienced supervisors who are outstanding professionals in their fields. Our health care team includes a variety of licensed specialties including physical, occupational and speech therapist, podiatrist, optometrist, audiologist, dentist, and psychologist.

Hospice care is available as well as respite care.

Carlisle Manor accepts Medicare, Medicaid, as well as private pay and insurance. Carlisle Manor is a VA contract provider of extended care.
